Dr. Stephen Kondash Departure Letter
Posted by: Midwest Eye Center in News
Dear Valued Patient:
After much thought and reflection, I have decided to retire from the practice of ophthalmology. Effective Oct. 1, 2026, I will no longer be practicing at MidWest Eye Center.
Since joining the practice in 1991, it has been my privilege to care for patients and families throughout the Cincinnati community for the past 35 years. Looking back on my career, I am filled with gratitude for the trust you placed in me and for the relationships we have built over the years. Caring for your vision and being part of your health care journey has been one of the greatest honors of my life.
While I will certainly miss seeing my patients and colleagues each day, I am looking forward to this next chapter of life, where I hope to spend more time with family and friends.
Although I am retiring, I want you to know that MidWest Eye Center remains committed to providing you with exceptional eye care. If you have been under my care, you are welcome to continue seeing another provider at your current location. Visit our website www.midwesteyecenter.com to learn more about our other providers. Your appointments will proceed as scheduled, and our team will contact you directly if any scheduling changes are necessary to ensure a smooth transition of your care.
If you have any questions or need assistance during this transition, our team is here to help. We are happy to assist with scheduling, medical records or any other support you may need.
You can request your medical records in any of the following ways:
| Online | Submit your request for medical records here: https://midwesteyecenter.com/patient-services/registration-forms/medical-records-release/ |
| Phone | Call us at 800-385-3937 to complete the form with our front desk staff. |
| In Person | Visit one of our office locations to complete a records transfer request: https://midwesteyecenter.com/locations/ |
Thank you for allowing me to be part of your lives and for trusting me with your eye care over the past 35 years. It has truly been a privilege to serve this community. I wish you and your loved ones continued health, happiness and all the very best in the years ahead.
Sincerely,
Stephen Kondash, M.D.
