To my valued patients,
After more than 40 years of practicing ophthalmology, I want to share that I will be retiring from patient care effective December 31, 2025. It has been a privilege to provide eye care to you and your family, and I am grateful for the trust you’ve placed in me throughout the years.
As I move into retirement, I take great comfort in knowing that your care will continue with the skilled doctors and staff at Midwest Eye Center. Our team of ophthalmologists and optometrists is dedicated to delivering the same high level of care and service that our patients have come to expect.
Your medical records will remain securely on file at Midwest Eye Center. If you wish to transfer them to another provider, our front desk staff can assist you with a records request form. You may also call 800-385- 3937 or visit our Hyde Park office at 2135 Dana Ave., Suite 310, Cincinnati, OH 45207 for assistance.
